Job summary

Housing Works Inc. is seeking a Care Manager to serve as a pivotal liaison among participants, staff, and health providers. You will coordinate access to medical and behavioral health services for those affected by HIV and Hepatitis C within a harm-reduction framework.

You will conduct intakes, perform risk assessments, provide education, and link clients to medical appointments, social supports, and treatment services in a supportive NYC environment.

Qualifications

• High school diploma or GED required.
• NYC DOH Peer Certificate Training or the ability to obtain certificate.
• HIV 101, Viral Hepatitis 101, Hepatitis C testing familiarity preferred.
• Experience in harm reduction, testing, and linkage to care.

Responsibilities

• Conduct intakes, risk assessments, and care planning to identify health needs.
• Build trusting relationships to help access resources and services.
• Coordinate HIV/HCV testing and linkage to care.
• Deliver health promotion and harm reduction education.
• Facilitate groups and educational activities safely and inclusively.
• Maintain accurate records and data entries in client databases.
