Job summary

Housing Works in New York, NY is seeking a Case Manager to conduct intake interviews, develop care plans, coordinate services and provide harm-reduction counseling to members with substance use and chronic health conditions. The role runs 9AM–5PM, Monday to Friday and involves collaboration with Ryan White and Harm Reduction prevention services.

Ideal candidates hold a Bachelor's in human services with 2+ years of case management experience and are bilingual in Spanish/English.

Qualifications

• Bachelor's degree in human services with at least two years of case management or social services experience.
• Minimum of a High School diploma with at least 5 years of case management experience.
• Ability to enter data into program databases such as AIRS, e-Share, EiCare.

Responsibilities

• Completes initial assessment and reassessments to develop individualized treatment goals.
• Establishes treatment goals with clients as part of the interdisciplinary team.
• Provides case management including progress summaries, care planning, referrals, and discharge planning.
• Facilitates harm reduction services and attends supervision meetings.
• Educates participants on Harm Reduction and Narcan administration.
• Coordinate with supervisors for life-saving services and field visits.
