Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with experience in primary care or internal medicine, preferably geriatrics, to work in nursing homes and assisted living communities enrolled in Medicare Advantage plans.

You will treat residents, coordinate care with case managers, and work under supervision to implement our national care model in compliance with regulations, delivering personalized, higher-quality care.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal agencies related to clinical services you provide.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ compliance and response to treatment and adjust plans accordingly.
• Collaborate with RN case managers as part of an integrated care team.
