Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with experience in primary care or internal medicine, preferably geriatrics-focused, to serve residents in long-term care settings. You will work within nursing homes or assisted living communities, delivering personalized, coordinated healthcare to improve quality of life and reduce hospital visits.

Qualifications

• Must be an APRN or PA licensed in good standing.
• Experience in geriatric primary care or internal medicine preferred.
• Electronic Health Records proficiency required.

Responsibilities

• Assess patients' medical and mental health needs and develop proactive care plans.
• Ensure compliance with healthcare regulations and facility policies.
• Prescribe medications and order labs, procedures, and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated team.
