Job summary

TruHealth is seeking an Advanced Practice Provider (NP or PA) with geriatrics experience to join our team serving residents in long-term care settings such as nursing homes or assisted living communities enrolled in Medicare Advantage plans.

Qualifications

• Requires APRN or PA license with clear credentials.
• 3+ years clinical nursing or rehab experience in geriatric populations.
• Experience with Electronic Health Records and Microsoft Office tools.

Responsibilities

• Assess patients and develop proactive care plans.
• Ensure compliance with local/state/federal regulations.
• Prescribe medications and order labs and tests.
• Monitor treatment progress and adjust plans as needed.
• Collaborate with RN case managers and care team.
