Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care or internal medicine and a geriatrics focus. You will work in long-term care settings, collaborating with a team of providers to improve outcomes for residents enrolled in Medicare Advantage plans.

Qualifications

• Master's degree or higher in Nursing or related field.
• APRN/PA license required or equivalent board certification.
• 3 years of geriatric clinical nursing or rehab experience.
• Experience with Electronic Health Records and standard office software.

Responsibilities

• Assess residents’ medical and mental health needs and develop proactive care plans.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor treatment response and modify plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
