Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with experience in primary care or internal medicine, preferably geriatrics, to improve outcomes for long-term care residents. You will work with a team of providers and case managers, with strong benefits and career growth opportunities.

Providers work mainly in nursing homes or assisted living communities enrolled in special Medicare Advantage plans, delivering coordinated care that improves life quality and reduces hospital visits.

Qualifications

• APRN or PA license required.
• Masters degree or higher.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Proficiency with Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and create a care plan addressing barriers to care.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
