TruHealth is seeking an advanced practice provider (NP or PA) with experience in primary care or internal medicine, preferably geriatrics, to join our team delivering care in long-term care communities.

You will work with a team of clinicians and case managers to improve outcomes for residents enrolled in Medicare Advantage plans, exercising independent judgment and coordinating with supervising physicians as needed.

Competenze

• FNP, AGNP, AHACNP or PA license required.
• Degree from accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• MS Office (Word, Outlook, Excel) knowledge.

Mansioni

• Assess patients' medical and mental health needs and create a care plan.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, procedures, and consults.
• Monitor patient progress and adjust treatment plans.
• Collaborate with RN case managers as part of the care team.
