Job summary

TruHealth is seeking an advanced practice provider, either an NP or PA, with primary care or internal medicine experience, preferably geriatrics. The role focuses on improving quality of life for long-term care residents in nursing homes or assisted living communities enrolled in Medicare Advantage plans.

As an APRN/PA, you will treat patients, coordinate care with supervising physicians, and work with facility partners to implement a national care model compliant with regulations.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ compliance and response to treatment and modify plans as needed.
• Work with RN case managers as part of an integrated care team.
