TruHealth is seeking an advanced practice provider (NP or PA) to deliver primary care and geriatric-focused services in long-term care settings. You will collaborate with a team of providers and case managers to improve outcomes for residents enrolled in Medicare Advantage plans.

You will practice with independent judgment under supervision, treating acute and chronic conditions while complying with federal and state regulations. A strong benefits and growth package is included.

Qualifications

• APRN/NP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications (Word, Outlook, Excel).

Responsabilités

• Assess patients’ medical and mental health needs and provide a plan of care that proactively manages conditions.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor patients’ response to treatment and modify plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
