Job summary

TruHealth is hiring an advanced practice provider (NP or PA) with geriatrics focus to improve long-term care outcomes. You will work within nursing homes or assisted living communities, collaborating with a team of providers to deliver coordinated care that enhances quality of life.

You will exercise independent judgment to treat acute and chronic conditions, coordinating with supervising physicians and health plan leadership to implement our national model while complying with regulations.

Qualifications

• Master's degree or higher required.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order labs, procedures and consultations.
• Monitor patient progress and adjust treatment as needed.
• Collaborate with RN case managers as part of an integrated care team.
