Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics or primary care experience to serve in nursing home and assisted living settings. You will deliver coordinated, quality care to residents enrolled in Medicare Advantage plans and work closely with a team of providers and case managers.

You will use independent judgment to treat acute and chronic conditions, collaborating with supervising physicians and facility leadership to implement TruHealth’s national model while

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and develop a care plan.
• Ensure compliance with local, state, and federal agencies related to clinical services.
• Prescribe medications and order labs, procedures and consultations.
• Monitor patients’ progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
