Job summary

AlignMed Partners seeks a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to float across Franklin Woods Center, Advanced Rehab at Autumn Lake, and Hammonds Lane Center in Baltimore, MD. The role offers a stable schedule with no on-call weekends.

You will provide primary and urgent care to nursing center residents, collaborate with the Center Leadership and Medical Director, and contribute to high-quality patient care. New grads welcome.

Qualifications

• Graduate from an accredited School of Nursing with RN license and NP certification (older adult/FNP).
• PA: ARC-PA accredited program and PANCE; PA licensure and DEA.
• Minimum two years experience with adult/geriatric population preferred.

Responsibilities

• Provide primary and urgent care to nursing center residents.
