Job summary

AlignMed Partners is seeking a Part Time Advanced Practice Provider (APP) to float between Autumn Lake Healthcare at Glade Valley Center and Ballenger Creek Center in Frederick, MD. 24–30 hours per week with no on-call or weekend requirements, eligible for a no-cap incentive program.

Applicants may be NP or PA; NP requires a Master’s in Nursing with a focus on Older Adult/Family practice, PA requires ARC-PA accreditation and PANCE, with state licensure and NCCPA certification.

Qualifications

• RN licensed
• NP with a Master's in Nursing and a focus on Older Adult or Family NP
• PA must graduate from an ARC-PA accredited program and pass PANCE and have state licensure

Responsibilities

• Provide primary and urgent care to nursing center residents.
• Collaborate with Center Leadership and Medical Directors on care plans.
• Report to Director of Clinical Practice or Area Clinical Practice Manager.
