Job summary

AlignMed Partners seeks an Advanced Practice Provider (NP/PA) to deliver primary and urgent care for residents across three NH centers: Laconia, Concord, and Franklin. The role offers 20–40 hours weekly with no on-call weekends and a competitive base salary between $120,640 and $133,120 per year, plus incentive compensation.

Requirements include NP or PA credentials, state licensure, and experience with geriatric patients.

Qualifications

• Graduate from an accredited nursing program with RN license and NP certification (Older Adult/Family NP concentration preferred).
• Or PA path: ARC-PA accredited program, PANCE, PA licensure, NCCPA certification.

Responsibilities

• Provide primary and urgent care to nursing center residents.
• Collaborate with Center Leadership and Medical Director on patient care.
• Support AlignMed Partners mission with high-quality care and compassion.
