Job summary

Provider Partners seeks an Institutional Special Needs Plan Nurse Practitioner to deliver coordinated care at designated skilled nursing facilities. You will work with the PCP to implement comprehensive care plans and serve as a medical liaison to the care team.

Requirements include active NP licensure in Missouri, DEA/CDS eligibility, BLS, and 3+ years of clinical experience with SNF/long-term care preferred.

Qualifications

• Licensed NP in Missouri with current credentials.

Responsibilities

• Demonstrates autonomy and proficiency in delivering comprehensive health care.
• Collaborates with the member's PCP to create a care plan.
• Adheres to PPHP documentation standards and credentialing requirements.
• Observes licensure scope and communicates with care team during transitions of care.
• Orders and interprets diagnostics, prescribes medications, and sets short/long-term goals.
• Provides patient and family education; serves as clinical liaison to staff.
