Job summary

Provider Partners Health Plan is seeking an Institutional Special Needs Plan Nurse Practitioner in Jefferson City, Missouri. The NP will deliver comprehensive care at designated skilled nursing facilities, working with the PCP to create preventive and chronic care plans while serving as a clinical liaison to the care team.

Responsibilities include autonomy in assessments, coordinating transitions, adhering to PPHP standards, and ensuring compliant documentation.

Qualifications

• Clinical competence with evidenced-based practice.
• Proficient computer skills; export data, create reports, and share results.

• Minimum 3 years clinical care experience; LTC/SNF preferred.
