Job summary

Provider Partners is seeking an Institutional ISNP Nurse Practitioner to deliver coordinated care for PPHP members in skilled nursing facilities, collaborating with primary care physicians and serving as the medical liaison to the care team.

The NP will assess, diagnose, order tests, prescribe when appropriate, develop a plan of care with short- and long-term goals, and support families and staff to ensure seamless, high-quality elder care.

Qualifications

• Clinical competence and understanding of evidenced-based practice.
• Proficient in computer skills and confident with new software.
• Good organizational and teaching skills.
• Strong customer service orientation and excellent communication.
• Minimum of 3 years clinical care experience.
• SNF/LTC experience preferred.
• Case management experience preferred.
• Knowledge of Medicare/Medicaid.
• Proficiency with I-SNP MOC and population.
• Proficiency in nursing/medical databases and Microsoft programs.

Responsibilities

• Demonstrates autonomy and proficiency in comprehensive health.
• Collaborates with primary care physician to create a care plan.
• Adheres to documentation standards.
• Observes state scope of practice.
• Communicates with physicians and family during transitions of care.
• Communicates with care team to formulate an appropriate plan of care.
