Job summary

Provider-Partners-Health-Plan- in Columbia, MO seeks an Institutional SNP Nurse Practitioner to deliver coordinated, comprehensive care in designated skilled nursing facilities, collaborating with PCPs to ensure seamless services.

You will perform preventive care, diagnose acute and chronic conditions, order tests, prescribe medications, create care plans with short- and long-term goals, and serve as a liaison to families and the care team across transitions of care.

Qualifications

• Clinical competence and understanding of evidenced-based practice.
• Proficient in computer skills- confidence in learning and using unfamiliar computer programs and electronic devices.
• Strong commitment to the care of the elderly and the needs of their families.
• Solid interpersonal and communication skills.
• Good organizational skills.
• Must be independent with autonomous problem-solving abilities.

Responsibilities
