Job summary

Provider Partners is seeking an Institutional Special Needs Plan (ISNP) Nurse Practitioner to deliver coordinated care at designated skilled nursing facilities. You will collaborate with primary care physicians, serve as a liaison to the interdisciplinary team, and ensure preventive and comprehensive health services.

The role requires independent decision-making, strong clinical skills, and effective communication with members, families, and staff to optimize outcomes within PPHP standards.

Qualifications

Licensed nurse practitioner with relevant scope of practice.
Experience with Medicare/Medicaid and I-SNP population is preferred.
Proficient in data export, reporting, and charting in EMR systems.

Responsibilities

Conduct comprehensive health assessments with autonomy.
Collaborate with PCP to develop prevention-focused care plans.
Adhere to PPHP documentation and care standards.
Observe state scope of practice; communicate during transitions of care.
Coordinate with care team to avoid duplications of tests.
Provide education to members, families and staff.
