Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to work in long-term care settings. You will deliver personalized, coordinated healthcare to residents enrolled in Medicare Advantage plans, collaborating with providers and case managers.

You will exercise independent judgment in treating acute and chronic conditions, with supervision as appropriate, and you will work with facility partners and health plan leadership to implement a national care model in

Qualifications

• Electronic Health Records experience.
• Experience with Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients' medical and mental health needs and create a care plan.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, procedures, and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of the care team.
