Job summary

TruHealth, part of American Health Partners, seeks an advanced practice provider (NP/PA) with geriatrics focus to join care teams in long-term care settings. You will provide personalized, coordinated primary care to residents enrolled in Medicare Advantage programs, working with facility partners and health plan leadership.

You will exercise independent judgement to treat acute and chronic conditions, consult supervising physician as needed, and participate in implementing our model of care in

Qualifications

• FNP/AGNP/AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients' medical and mental health needs and plan care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics, and consults.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of the care team.
