TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to join a team caring for long-term care residents. You will work with providers and case managers to deliver coordinated, high-quality care in nursing homes or assisted living settings enrolled in Medicare Advantage plans.

As an APRN or PA, you will exercise independent clinical judgment, treat acute and chronic conditions, and collaborate with facility partners and plan leadership to implement our national

Formación

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsabilidades

• Assess and manage medical and mental health needs of residents with proactive care plans.
• Ensure compliance with local, state, and federal regulations applicable to clinical services.
• Prescribe medications and order labs, imaging, and consults as appropriate.
• Monitor treatment response and adjust plans to improve outcomes.
• Collaborate with RN case managers as part of an integrated care team.
