Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric focus to join our team in long-term care settings. You will deliver personalized, coordinated health care to residents enrolled in special Medicare Advantage plans, working closely with facility partners and health plan leadership.

You will exercise independent judgment to treat acute and chronic conditions, while consulting a supervising physician as needed.

Qualifications

• Masters degree or higher in nursing or related field.
• APRN/PA license required (FNP, AGNP, AHACNP or PA).
• Minimum 3 years’ experience in geriatric clinical nursing or rehab.

Responsibilities

• Assess patients’ medical and mental health needs and create care plans.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications, order labs, and coordinate diagnostics and consultations.
• Monitor patient response to treatment and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
