TruHealth is seeking an advanced practice provider (NP or PA) with geriatric focus to improve outcomes for long-term care residents. You will work with a team of providers and case managers to deliver proactive, coordinated care within nursing homes or assisted living communities enrolled in Medicare Advantage plans.

You’ll exercise independent judgement to treat acute and chronic conditions, collaborating with facility partners and health plan leadership to implement our care model in

Formación

• APRN/PA licensed with applicable board certification.
• Master's degree or higher from an accredited program.
• Minimum of 3 years of geriatric clinical nursing or rehab experience.

Responsabilidades

• Assess patients and plan proactive care for geriatric residents.
• Coordinate with facility staff and health plan leadership.
• Prescribe medications and order labs as appropriate.
• Work with RN case managers as part of a care team.
• Ensure compliance with federal/state regulations.
