Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric focus to improve long-term care outcomes. You will work with a team of providers to deliver personalized, coordinated care for residents enrolled in Medicare Advantage plans.

You will practice with independence under supervision, in nursing homes or assisted living settings, contributing to quality of life improvements and reduced hospital visits.

Qualifications

• APRN/NP license required.
• Masters degree or higher in nursing or related field.
• 3 years’ experience in geriatric clinical nursing or rehab.
• Experience with Electronic Health Records (EHR).
• Proficiency with Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, imaging and consults.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of the care team.
