TruHealth is recruiting an advanced practice provider (NP or PA) with geriatrics experience to enhance care in long-term care facilities. You will work with a team of providers and case managers to deliver coordinated, high-quality health services to residents enrolled in Medicare Advantage plans.

The role emphasizes independent clinical judgment, collaboration with facility partners, and compliance with federal/state regulations while offering a strong benefits package and meaningful career

Formación

• Masters degree or higher required.
• APRN/PA licensure required (FNP/AGNP/AHACNP or PA).
• Degree from an accredited APRN or PA program.

Responsabilidades

• Assess patients' medical and mental health needs and develop a proactive plan of care.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics, and consults.
• Monitor treatment and adjust plans as needed.
• Collaborate with RN case managers as part of the care team.
