Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to deliver coordinated primary care in long-term care settings. You will work with a team of providers to improve outcomes for residents enrolled in Medicare Advantage plans.

You will exercise independent judgement in treating acute and chronic conditions, collaborating with supervising physicians and facility partners to implement a national care model compliant with laws and regulations.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal agencies related to clinical services.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
