Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics and primary care experience. You will work in nursing homes or assisted living communities to coordinate personalized health care, improve outcomes, and support residents.

Providers work with supervising physicians, case managers, and health plan leadership to implement care models and comply with regulations, enjoying a strong benefits and growth package.

Qualifications

• APRN or PA license
• Masters degree or higher in nursing or related field
• 3 years’ experience in geriatric clinical nursing or rehab
• Electronic Health Records experience
• Proficiency with Word, Outlook and Excel

Responsibilities

• Assess patients’ medical and mental health needs and develop proactive care plans
• Ensure compliance with local, state, and federal regulations governing clinical services
• Prescribe medications and order labs, diagnostics, and referrals
• Monitor patients’ response to treatment and adjust plans as needed
• Collaborate with RN case managers as part of an integrated care team
