Job summary

TruHealth seeks an advanced practice provider (NP or PA) with geriatrics focus to improve long-term care outcomes by coordinating with a team of providers and case managers. You will work in nursing homes or assisted living communities enrolled in Medicare Advantage plans, contributing to a coordinated, quality care model.

You will treat acute and chronic conditions with independent judgment, under physician supervision as needed, and help implement our national care model in compliance with

Qualifications

• APRN/PA license required and board certification preferred.
• 3 years’ geriatric clinical nursing or rehab experience.
• Experience with electronic health records.
• Proficient with MS Office suite.

Responsibilities

• Assess patients and develop proactive care plans.
• Ensure regulatory compliance in clinical services.
• Prescribe medications and arrange labs/consults.
• Monitor treatment outcomes and adjust plans.
• Collaborate with RN case managers within an integrated team.
