Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to deliver proactive, coordinated care in nursing homes or assisted living communities. You will work with a team of providers and case managers to improve quality of life for seniors enrolled in Medicare Advantage plans.

You will exercise independent judgement in treating acute and chronic conditions, with supervision as appropriate, and participate in implementing our national model of care while complying

Qualifications

• Masters degree or higher in nursing or related field.
• APRN or PA license required; geriatrics/long-term care experience preferred.
• 3 years’ experience in geriatric clinical nursing or rehab.
• EHR experience; MS Office familiarity.

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal agencies for clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with RN case managers as part of the integrated care team.
