Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric primary care experience to improve outcomes for residents in long-term care settings.

You will collaborate with a team of providers and case managers to deliver coordinated, patient-centered care, prescribe medications, and order necessary tests while aligning with our national care model and regulatory requirements.

Qualifications

• Masters degree or higher in nursing or PA studies.
• APRN/PA licensure and certification.
• 3 years clinical nursing or rehab experience in geriatrics.
• Experience with EHRs and MS Office.

Responsibilities

• Assess patients and develop care plans for geriatrics.
• Ensure compliance with regulations at local, state and federal levels.
• Prescribe medications and order labs and referrals.
• Collaborate with RN case managers as part of an integrated team.
• Monitor treatment outcomes and adjust plans as needed.
