Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care or internal medicine and a geriatrics focus. This role offers the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers, with a competitive benefits package.

Qualifications

• APRN or PA license required and APRN/PA program degree.
• Experience in primary care or internal medicine with geriatrics focus preferred.
• Strong care coordination and team-based approach.

Responsibilities

• Assess patients' medical and mental health needs and plan care.
• Ensure compliance with local, state, and federal agencies related to clinical services.
• Prescribe medications and order labs, diagnostic procedures and consultations.
• Monitor patients' response to treatment and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
