Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with geriatrics experience to improve outcomes for long-term care residents. You will collaborate with a team of clinicians and case managers to deliver personalized, coordinated healthcare within nursing homes or assisted living facilities enrolled in Medicare Advantage plans.

You will exercise independent judgement, manage acute and chronic conditions, and work with facility partners to implement TruHealth’s national care model while

Qualifications

• Licensed NP or PA with geriatric focus preferred.
• Degree from APRN or PA program required.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.

Responsibilities

• Develop and implement care plans for long-term care residents.
• Prescribe medications and order labs and imaging as needed.
• Coordinate with RNs and health plan teams to ensure integrated care.
