Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to deliver coordinated primary care in nursing homes and assisted living facilities. You will work with a team of providers and case managers to improve outcomes for long-term care residents within our national model of care.

The role emphasizes independent clinical judgment, adherence to regulations, and collaboration with facility partners.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a care plan.
• Ensure compliance with local, state, and federal agencies related to clinical services.
• Prescribe medications and order lab work, diagnostics and consultations.
• Monitor patient compliance and response to treatment.
• Collaborate with RN case managers as part of an integrated care team.
