Job summary

TruHealth is seeking an advanced practice provider (NP/PA) to deliver primary care to seniors in long-term care settings. You will collaborate with a team to improve outcomes and manage chronic conditions, focusing on preventative care and reducing hospital visits.

You will work with facility partners and health plan leadership, prescribing medications, ordering tests, and coordinating with RN case managers to implement our model of care in compliance with regulations.

Qualifications

• APRN or PA license required, state-licensed in Indiana preferred.
• Master's degree or higher in nursing or related field.
• 3 years' experience in geriatric care or rehab.
• Experience with Electronic Health Records.
• Proficient with Microsoft Word, Outlook, Excel.

Responsibilities

• Provide primary care to long-term care residents.
• Prescribe medications and order labs, imaging, and consultations.
• Collaborate with RN case managers and supervising physician.
• Ensure compliance with local, state, and federal regulations.
