Job summary

TruHealth is seeking an advanced practice provider (NP/PA) to lead primary care in long-term care settings, focused on improving resident outcomes through collaborative care with a multidisciplinary team.

Ideal candidates have geriatric experience, APRN or PA licensure, and proficiency with EHRs. You will join a team delivering coordinated healthcare in nursing homes and assisted living communities under Medicare Advantage plans.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• MS Word, Outlook and Excel proficiency.

Responsibilities

• Assess medical and mental health needs; create proactive care plans.
• Ensure compliance with local, state, and federal agencies.
• Prescribe medications and order labs and diagnostics.
• Monitor treatment effectiveness and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
