Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to enhance care for residents in long-term care settings. You will work with a multidisciplinary team to deliver coordinated health care that improves quality of life and reduces hospital visits.

The role emphasizes independent clinical judgment within senior care and collaboration with facility partners and health plan leadership to implement TruHealth’s model in compliance with regulations.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and develop care plans.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order lab work, diagnostics and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
