Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics-focused primary care to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers in nursing homes or assisted living communities.

You will practice with independence while consulting a supervising physician as needed, contributing to our national model of care aligned with regulations. Excellent benefits and growth opportunities are offered.

Qualifications

• FNP, AGNP, AHACNP or PA license required
• Degree from an accredited APRN or PA program
• 3 years’ experience in clinical nursing or rehab in geriatric populations
• Electronic Health Records experience
• Working knowledge of Microsoft applications, including Word, Outlook and Excel

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care that proactively manages their conditions
• Ensure compliance with local, state, and federal agencies related to clinical services
• Prescribe medications and order lab work, diagnostic procedures and consultations
• Monitor patients’ compliance and response to their treatment and modify those plans
• Work with RN case managers as part of an integrated care team

Education

Masters degree or higher in APRN (FNP/AGNP/AHACNP) or PA program

APRN/PA license required

Electronic Health Records experience

MS Office (Word/Outlook/Excel) experience
