Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to deliver primary care within long-term care settings. You will collaborate with a team of providers and case managers to improve resident outcomes and quality of life.

Applicants should hold an APRN or PA license and a degree from an accredited program; 3 years of geriatric clinical experience is preferred. The role emphasizes independence, coordination with facility staff, and compliance with regulations.

Qualifications

• APRN/NP or PA license required.
• Degree from an accredited APRN or PA program.
• Masters degree or higher preferred.
• Electronic Health Records experience.
• Proficiency with Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients' medical and mental health needs and develop care plans.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor treatment progress and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
