Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with geriatrics experience to improve outcomes for long-term care residents. You will work with a team of providers and case managers to deliver coordinated care in nursing homes or assisted living communities enrolled in Medicare Advantage plans.

You will exercise independent judgement to treat acute and chronic conditions, while ensuring compliance with federal and state regulations and collaborating with facility partners and health

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in geriatric clinical nursing or rehab.
• Experience with Electronic Health Records and MS Office.

Responsibilities

• Provide care plans for geriatric residents and coordinate with team.
• Prescribe medications and order labs, tests, and referrals.
• Monitor treatment progress and adjust plans as needed.
• Collaborate with RN case managers and facility staff.
