Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with a geriatrics focus to enhance care for long-term residents in nursing homes or assisted living facilities.

You will collaborate with a multidisciplinary team to deliver personalized, coordinated health care, comply with federal and state regulations, and leverage our national model of care to improve patient outcomes and reduce hospital visits.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and develop a care plan.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor patient compliance and adjust treatment plans.
• Collaborate with RN case managers as part of an integrated care team.
• Ensure compliance with local, state, and federal regulations.
