Job summary

Total Life is seeking a nurse practitioner in the Grand Junction area for our Longevity program. You will lead on-site longevity days at partner senior living communities, starting with an educational talk and followed by one-on-one visits with residents.

You'll be trained by our Chief Medical Officer, a board-certified geriatrician, and your first day will be co-run with a team member. Site days are weekdays, with hours 4–8 per day and increasing as bookings grow.

Qualifications

• Active Colorado APRN registration and RN license.
• Full prescriptive authority in Colorado, or provisional with mentoring.
• Current national NP board certification.
• Active DEA registration and PDMP registration.
• Clinical experience caring for adults 65+.
• Experience with Medicare documentation and E/M coding.

Responsibilities

• Conduct 45–60 minute longevity visits based on comprehensive geriatric assessment.
• Evaluate eligibility for weight management, including GLP-1 therapy when appropriate.
• Coordinate follow-up care with primary care providers and specialists.
• Document encounters accurately per Medicare standards in the EHR.
• Represent Total Life professionally with residents, families and staff.
• Travel to assigned senior living communities in the Grand Junction area.
