Job summary

Total Life is seeking an in-person nurse practitioner to lead longevity days at partner senior living communities in the Joliet-Palatine area. You will conduct comprehensive geriatric assessments, perform longevity visits, and coordinate with residents’ primary care teams.

You will be trained by our CMO and will visit multiple sites on weekdays with hours that grow as bookings increase. This contractor role emphasizes geriatric care and group presentations.

Qualifications

• Active, unrestricted Illinois APRN license and RN licensure.
• Current national NP board certification and DEA registration.
• Experience with Medicare documentation and E/M coding.
• Ability to work as a contractor in the Chicago suburbs area.
• Comfort presenting to groups and families of older adults.

Responsibilities

• Conduct 45-60 minute longevity visits after an educational talk.
• Coordinate with primary care providers for follow-up care.
• Document encounters to Medicare standards in the EHR.
• Travel to assigned senior living communities in the Joliet-Palatine area.
• Represent Total Life professionally to residents, families, and staff.
