Total Life, Inc. is seeking an experienced Nurse Practitioner to lead Longevity days at partner senior living communities in the West Valley (Phoenix).

This in-person contractor role focuses on comprehensive geriatric assessment, medication review, and cognitive health visits with residents who sign up for one-on-one longevity visits. You'll be trained by our CMO, work with a team, and travel between sites.

Qualifications

• Active AZ APRN with prescribing authority and ARNP license.
• Current national NP board certification and DEA registration in AZ.
• Experience caring for adults 65+ and Medicare documentation.
• Own malpractice insurance covering in-person care or ability to obtain it before first site day.
• Weekday on-site availability in the West Valley (Phoenix) area.

Responsabilités

• Conduct 45–60 minute longevity visits after a comprehensive geriatric assessment.
• Evaluate weight management and GLP-1 therapy when appropriate.
• Coordinate follow-up care with primary care providers.
• Document encounters to Medicare standards in our EHR.
• Represent Total Life professionally with residents, families and staff.
• Travel to assigned senior living communities in the West Valley.
