Job summary

Total Life, Inc. in the Phoenix area seeks nurse practitioners for in-person longevity visits at partner senior living communities. Each site day includes a 45–60 minute assessment, medication review, and cognitive screening, with training from a geriatric physician.

Work is on weekdays, 4–8 hours per day as bookings grow, with mileage reimbursement and flexible scheduling. You’ll coordinate care with primary providers and document to Medicare standards in our EHR.

Qualifications

• Active, unrestricted Arizona APRN certificate and RN license with prescribing/dispensing authority.
• Current national NP board certification.
• Active DEA registration and State Board of Nursing registration for controlled substances.

Responsibilities

• Conduct 45–60 minute longevity visits with geriatric assessment, medication review, cognition and functional screening.
• Evaluate for weight management and GLP-1 therapy where appropriate.
• Identify follow-up needs and coordinate with residents’ primary care providers.
