Total Life in North San Diego County is seeking a Nurse Practitioner to lead longevity days at partner senior living communities. You will conduct one-on-one visits after an initial education talk, supported by our CMO, and travel to assigned sites.

The role emphasizes geriatric assessment, cognitive screening, medication review, and collaboration with primary care, with a flexible contractor schedule and ongoing program development.

Qualifications

• Active, unrestricted California RN license and NP certification from the California BRN.
• Active NP Furnishing Number from the California BRN.
• Current national NP board certification.
• Active DEA registration for CA and CURES enrollment.
• Ability to practice under standardized procedures with a supervising physician.
• Clinical experience caring for adults 65+.
• Experience with Medicare documentation and E/M coding.
• Own malpractice insurance (min $1M per occurrence, $3M aggregate) for in-person care.
• Valid driver's license and reliable transportation.
• Weekday availability for on-site days in the North San Diego County area.

Responsabilités

• Conduct 45–60 minute longevity visits based on a geriatric assessment.
• Evaluate weight management options, including GLP-1 therapy where appropriate.
• Identify residents needing follow-up care and coordinate with their PCP.
• Document encounters accurately to Medicare standards in our EHR.
• Represent Total Life professionally with residents, families and staff.
• Travel to assigned senior living communities in North San Diego County.
