Job summary

Total Life, Inc. is seeking nurse practitioners for an in-person contract role in the West Valley (Phoenix) area to lead longevity days at partner senior living communities.

Each day starts with a short educational talk for residents on healthy aging, followed by one-on-one longevity visits. You will be trained by our Chief Medical Officer and responsible for geriatric assessments, medication reviews, and coordinating follow-up care with primary providers.

Qualifications

• Active, unrestricted AZ APRN license with prescribing authority.
• Current national NP board certification.
• Experience with Medicare documentation and E/M coding.

Responsibilities

• Conduct longevity visits with a comprehensive geriatric assessment.
• Evaluate residents for weight management and GLP-1 therapy where appropriate.
• Identify and coordinate follow-up care with primary care providers.
• Document encounters accurately to Medicare standards in the EHR.
• Travel to assigned senior living communities in the West Valley.
