Total Life is seeking nurse practitioners to lead longevity days at partner senior living communities in the Porterville-Bakersfield area. You will present and conduct 45-60 minute visits after a training session with our CMO, who designed the program's clinical content.

Responsibilities include geriatric assessments, medication review, cognitive screening, and coordinating follow-up care with residents’ primary providers. Site days are weekdays with travel between communities.

Qualificações

• Active, unrestricted California RN license.
• California NP certification and Furnishing Number.
• Current national NP board certification.
• DEA registration for California and CURES.
• Ability to practice under standardized procedures with a supervising physician.

Responsabilidades

• Lead longevity visits with a geriatric assessment and deprescribing as needed.
• Travel to Porterville-Bakersfield senior communities for site days.
• Document encounters to Medicare standards in our EHR.
• Represent Total Life professionally to residents, families and staff.
