Job summary

Total Life Inc. is seeking nurse practitioners for an in-person contract role in the Grand Junction area to lead longevity days at partner senior living communities.

Each day begins with an educational talk, followed by one-on-one longevity visits, training and support from our Chief Medical Officer who designed the program content. Site days are on weekdays; expect 4–8 hours per site day, a few times per month to start, with hours growing as bookings rise.

Qualifications

• Active, unrestricted listing on the Colorado APRN Registry as a nurse practitioner with the Colorado Board of Nursing.
• Full prescriptive authority in Colorado, or provisional prescriptive authority with the ability to complete the required 750-hour prescribing mentorship.
• Current national NP board certification.
• Active DEA registration and registration with the Colorado Prescription Drug Monitoring Program.
• Experience with Medicare documentation and E/M coding.
• Maintaining own malpractice insurance (min $1M per occurrence, $3M aggregate) covering in-person care.

Responsibilities

• Conduct 45 to 60 minute longevity visits built on a comprehensive geriatric assessment.
• Evaluate eligible residents for weight management and GLP-1 therapy where appropriate.
• Identify residents needing follow-up care and coordinate with their primary care provider.
• Document every encounter accurately to Medicare standards in the EHR.
• Represent Total Life professionally with residents, families and staff.
• Travel to assigned senior living communities in the Grand Junction area.
